Insurance and Payment
Our physicians are participating providers with most major health insurers including Medicare. To learn if we are providers on your insurance plan, please call our office and ask for Liz Seldon, our office manager.
We require payment of deductibles and co-pays at the time of service.
Information for your Office Visit
Please bring with you the following: your insurance card, a list of all current medications including dosages, any previous cardiology medical records
If you are a new patient, please arrive 30 minutes early to fill out paperwork and have ECG done. If you are an existing patient, please arrive 15 minutes early
